Final date for applications – 22nd January 2010

 NHS South Gloucestershire is reviewing all applications for Continuing Healthcare which were made after October 2007.  We are trying to contact relatives of those people who sadly died before an initial assessment or review of their healthcare needs was completed to see if they wish to request a retrospective review assessment as you may be entitled to a refund of costs.
