If you are living with a significant or complex health need that means you require care provided over an extended period of time to meet physical or mental health needs, you may be eligible to receive NHS continuing healthcare and NHS-funded nursing care.

This funding is available to adults over the age of 18 who have been assessed using a process that is outlined in the Department of Health’s National Framework for NHS Continuing Health Care and Nursing Care and have been deemed eligible for funding.

How do I know if I am eligible?
The funding takes into account the nature, complexity, unpredictability or intensity of a person’s care needs. This is considered within a comprehensive assessment process which starts with:

Initial Screening
This involves the completion of a standard initial screening form. This form gives us an understanding of whether you are eligible for referral into NHS South Gloucestershire’s continuing healthcare team for full assessment

Referral for full assessment by a continuing healthcare assessor
The continuing healthcare assessor will work with your family and primary healthcare team to collect information which enables the case to be viewed in more detail according to the national guidance.

How is funding provided?
If you are found eligible for funding it may be provided in two ways:

Continuing Healthcare (CHC)
Under continuing healthcare the NHS arranges and fully funds a package of health and social care to meet your needs. This can be provided in a number of settings including hospital, your own home and care home environments. The decision about where care is provided depends upon your wishes and/or the practicalities of providing adequate and appropriate care.

Funded Nursing Care (FNC)
If you are living in a nursing care home, and found eligible for funded nursing care, the NHS will provide a contribution towards the funding of registered nurse care.
